Hard to say. Maybe this photo:

I think it was taken in 1915.

Good question. However, the whole IF was photographed in Tobolsk in early 1918, I don't know if those shots were formal ones (According to the movie ' Romanovy- Ventsenosnaya Semya', those shots were formal, I believe in Russian sources), but indeed the photo of Tsarevich and his father was taken in late 1915, before Alexei's almost -fatal nose-bleeding prior to 1916 (late, in December of 1915).  1916 formal shots of his sisters do exist, however, I am not aware of any formal of him from 1916. He only appeared in Russian military magazines/newspapers during the WW1. but remember his photos before the cannon and the tree, taken in 1916? Also, may we label these as 'formal'?

Thank you all! I really wanted a picture of Alexis to draw, and I wanted it to be his last formal photo. Smiley

Uh, it is hard to tell, I have seen one labeled to have been painted in 1917 :

Yes, tsarevich was painted several times by some artist during Nicholas' reign.
