The diary is from KR, dated March 18, 1912. There is no indication where the lunch took place. As his diary is dated Pavlovsk, I would surmise that the meal took place at either Alexander Palace or Pavlovsk. The diary entry I have is from A Life Long Passion: Nicholas and Alexandra Their Own Story.
As for the meal manners...any childish behavior of the Imperial children didn't fall far from the tree. In Coryne Hall's book, Little Mother of Russia, A Biography of Empress Marie Feodorovna, the author states in Chapter 5, The Cross and the Crescent...
"They were probably among the least restricted and disciplined Romanov children. Dagmar taught them that family life was important and, because of their parent' happy marriage, the were brought up in an atmosphere of love and security missing from most royal households. As they grew older (Nicholas and his siblings) they dined with their parents and these occasions frequently turned into battles as the children, in fits of laughter, pelted each other with bread. Sasha's aunt, Queen Olga of Wurttemberg, was appalled at his family's table manners."
If there is any information on the table manners of Nicholas and Alexandra's children, other than the recorded event of Alexei, I haven't come across it.
