And Alexandra did smoke. She write to Nicholas she stopped smoking during the war. She used to smoke only French cigarettes when she was alone to calm down her nerves...
Cigarettes were considered as a sedative and a expectorant at this time. I even saw advertising where cigarettes were sponsored by the Medicine University!

This is the dress of Alexandra. This mantel was made in 1896 for the coronation.
