Nicholas II's attempt to have a private secretary in 1905, similar to his father in the 1880s, was thwarted:

I can't imagine a head of state, in this case an Emperor of Russia not having a secretary. And I think this would apply whether the role of said secretary was merely clerical and mundane or if it entailed the expectation s of being a confidante/ advisor.
In any case, Nicholas' foregoing a secretary met with little approval, to put it mildly.
